Best Monitor in Action: best practices of selfmanagement and shared decision making in remote care

The demand for healthcare is increasing faster than the supply, placing significant pressure on healthcare professionals. Digital healthcare offers opportunities to maintain the quality of care while addressing capacity challenges. An innovative approach in this context is home monitoring, carried out by monitoring nurses in a specialized role that has not yet been systematically evaluated. 

Aim

This project aims to identify best practices for shared decision-making and self-management support in home monitoring provided by monitoring nurses. Self-management support and shared decision-making are concepts that align closely with the principles of personalized and appropriate care. 

Approach

The interventions and the conditions necessary for successful implementation will be documented in an implementation guide, which will serve as a tool for scaling this new approach in both secondary and primary care settings. Additionally, the implementation guide can form a foundation for professional education.
