Reconnect patients after cancer treatment with primary care for further rehabilitation

After cancer treatment, patients undergo aftercare and checks in the hospital. Despite the availability of the general practitioner (GP) as a primary contact for cancer patients, it remains unclear to many patients where they can seek care after their cancer treatment. Sometimes patients experience problems long after treatment. Due to demographic developments, we expect an increase in the number of cancer patients, necessitating more efficient organization of care.

Aftercare Decision Tool

By establishing a clear reconnection with the GP, patients and GPs will have a better understanding of which care services can be accessed in primary care, while checks stay in the hospital. An existing decision tool for breast cancer supports discussions between patients and GP about recovery and helps decide the most appropriate aftercare.

Aim 

In this project, we will develop the Aftercare Decision Tool for prostate cancer and subsequently implement and evaluate both Aftercare Conversation Tools in primary care.

General Practitioner care research

We need more knowledge from research into general practice, and we need to disseminate this knowledge among GPs in order to maintain the current high standards of research and care. We therefore support academicization by funding PhD research projects for GPs training to become clinical researchers.
