Reduction of crisis admissions in persons with dementia via digital monitoring of informal caregiver

‘Small crises’, such as falls, wandering, and behavioral problems, can negatively impact the resilience of informal caregivers. This can lead to crisis admissions.

Digital monitor REMIND

To detect ‘small crises’ and decline in caregiver resilience early, the digital monitor REMIND was developed in collaboration with family caregivers and healthcare providers. REMIND allows caregivers to report weekly on ‘small crises’ and their resilience. Case managers or district nurses can view this information on a dashboard.

Aim

The aim of this project is to reduce crisis admissions for people with dementia.

Approach

In this project, the effects of REMIND are evaluated on the number of crisis admissions. They also adapt REMIND for people with limited health literacy and other target groups, as caregivers for people with Parkinson's or a stroke. Additionally, they develop an e-learning and a guide to support case managers and district nurses in providing appropriate care for people with dementia and their caregivers.
