Telenurse: description and evaluation of the new position of nurse telecoach

The Digital Health Center (DHC) helps people with chronic illnesses live healthier and more independent lives. We achieve this by using digital care solutions, such as home monitoring and online consultations. A dedicated nurse, the telecoach, supports patients not only medically but also in areas like mental health, quality of life, and social issues. Since 2023, more than 1,500 patients have been using the DHC.

More efficient and sustainable healthcare

The DHC collaborates with hospitals, general practitioners, and other healthcare organizations. This approach reduces the need for patients to visit the hospital and gives them more control over their own care. This is not only beneficial for patients but also makes healthcare more efficient and sustainable. 

Aim

We are studying how the telecoach coordinates care, what this means for patients, and how it changes the work of healthcare providers. We are also developing a training program for nurses. Our approach could serve as a model for other regions in the Netherlands.

Collaboration

The Digital Health Center (DHC) is an innovative collaboration between the Red Cross Hospital, ViVa! Zorggroep (longterm care), General Practitioners in Midden- and Zuid-Kennemerland, and Heliomare (rehabilitation care).
